All Years:

• Financial Aid Addendum - Confidentiality Waiver
• Texas Residency Form -
  • Short Version - for students that have lived in Texas for the past 36 months and graduated from a Texas high school.
  • Long Version - for all other students.
  • Affidavit - print only if required for your Texas Residency Form.
• Request for Copy of Tax Return (IRS Form 4506) - The IRS charges a fee of $39.00 for a copy of your tax return.  Please see the information below regarding transcripts of tax returns before completing your request.
• Request for Transcript of Tax Return (IRS Form 4506-T) - Unless otherwise noted, a transcript of your tax return will provide sufficient information for our office.  The IRS does not charge a fee for transcripts of tax returns.  For more information about this form, you can also call the IRS at 1-800-829-1040.
