The forms listed below can be downloaded, printed, and returned to the Registrar's Office for processing. You may bring the forms to the Office of Registration and Records in Beck Center, Suite 110, during normal business hours, or you may fax the forms to 830-372-8179. Forms may also be  mailed to the following address:
