
Located on this page are forms that athletes are required to fill out in order to participate in athletics at Texas Lutheran. If you have not filled out all of the forms listed below and located on this page prior to the first day of practice you will not be able to begin practice with your athletic team. If you have any questions about the paper work required to participate in athletics, please contact your coach or the athletics secretary Judy Johnson (judyjohnson@tlu.edu).
**ATTENTION NEW ATHLETES**
The following forms are to be filled out only by new incoming athletes: Pre-participation Physical and Authorization to Release Medical Information. Along with those two forms, the following are required as well: Insurance Questionnaire, HIPPA, Proof of Loss, and Insurance Policy Form. Finally, a copy of your insurance card is also required.
**RETURNING ATHLETES**
Each year returning athletes are required to fill out the following forms: Insurance Questionnaire, HIPPA, Proof of Loss, and Insurance Policy Form. A copy of your insurance card is also required.

**NEED INSURANCE???**
Students at Texas Lutheran are require to carry comprehensive medical insurance in order to participate in athletics. If you are not covered under a comprehensive medical insurance plan, you need to obtain a policy that covers intercollegiate athletics, allows for providers in the Seguin-New Braunfels area, and has a maximum coverage limit of $75,000 or better per incident.
